First-Time Anterior Dislocators: What to Do and How to Do it

Jim Bradley, MD, (Pittsburgh, PA) discusses the incidence of first time anterior dislocators and the predictors of recurrence based upon their age, activity level, sport, sex, and injury. He describes the pathologic sequence of this injury including bony Bankart lesions, capsular deformation and elongation, Hill Sachs lesions, isolated capsular tear, and HAGL lesions and their effect on recurrent dislocation. He also shares his decision-making algorithm including non-operative management, contraindications, surgical intervention, patient selection criteria, and technique pearls.
