The Netherlands is the only authorized EU Port of Entry for Arthrex import shipments.</p><p>Suppliers located outside the European Union that ship Prototypes, Samples, Repairs and other Non-Inventory items must refer to the <a href="https://www.arthrex.com/link/asset/zrW1pzP--keAGwF1ReyYzw" target="_blank" rel="noopener">Process Import Prototypes, Samples, Repair and Non-Inventory Shipments</a> instructions.</p><p>If you have questions regarding Customs, or other government agency import requirements, then contact <a href="mailto:GmbHDepartmentCustoms&amp;Trade@arthrex.de">GmbHDepartmentCustoms&amp;Trade@arthrex.de</a>.</p><p><strong>EU Customs Requirements</strong></p><p>NL Customs requires a complete and accurate English language declaration of the goods.</p><p>As an Arthrex supplier, you must provide the below information on your Commercial Invoices and shipment summaries to enable Arthrex to import your products. This information is mandatory:</p><p>Commercial Invoice</p><ul><li>Invoice number, terms of sale</li><li>INCOTERMS / Terms of delivery</li><li>Shipper / exporter&rsquo;s full name, address and tax identification number</li><li>Ship-to name and address as instructed by the Buyer, and specified on the purchase order</li><li>Arthrex purchase order number</li><li>Arthrex part number as shown on the purchase order</li><li>A complete and accurate description of each product</li><li>Harmonized Tariff Schedule (HTS) classification number</li><li>Currency</li><li>Country of Origin (COO), and upon request, provide a Certificate of Origin positively affirming the country of origin of all materials.<ul><li>If the Supplier changes the country of origin of any item it sells to Arthrex, it must immediately provide a written summary of the changes to the Global Supply Chain Buyer.</li></ul></li><li>Product Value &ndash; must reflect the purchase price of each item; if you send an item at no charge, then include its cost, or the Fair Market Value of a similar item.</li><li>Include any additional value for:<ul><li>Components or materials provided by Arthrex at a discount or free, (e.g. packaging materials, suture components, and implant components)</li><li>Customized or special packaging that exceeds normal packing or packaging costs.</li></ul></li><li>Arthrex requires suppliers located within the European Union to send a Long Term-Supplier Origin Declaration for products with or without preferential origin. Send a PDF version of the Declaration via email and the original version via postal mail to Arthrex Global Trade and Customs department, Erwin-Hielscher-Str.9, 81249 Munich.<ul><li>The Supplier Declarations must arrive before the first delivery of each year or, in case of single Supplier declaration, at the time of each delivery.</li></ul></li><li>Arthrex requires non-European Union suppliers to provide a Certificate of Origin with each delivery, or alternatively, declare preferential origin on the Commercial Invoice.</li></ul><p><strong>Export Compliance</strong></p><p>The Supplier is obligated for export compliance purposes to provide the following information for the items the Supplier delivers:</p><ul><li>Export list number according to Annex AL to the Foreign Trade and Payments Ordinance or similar list of items of relevant export lists;</li><li>Country of origin for its goods and any component thereof, including technology and software, for purposes of trade policy;</li><li>For U.S. goods, the ECCN (Export Control Classification Number) or license number EAR 99, according to the U.S. Export Administration Regulations;</li><li>If the goods were transported, manufactured or stored in the U.S. or made using U.S. technology;</li><li>Contact person within its company that will answer Arthrex&rsquo;s questions</li></ul><p>At Arthrex&rsquo;s request, and prior to order fulfilment, the Supplier must provide a contact person at their company responsible for orders and any additional export information regarding supplied goods and their components.</p><p>&nbsp;</p></div></article></section></section></div></div></section>
